I founded Courtyard Nutrition & Therapy Associates in 2004 to bring together nutritionists and psychotherapists with similar interests. Housed in a warm and inviting garden suite in Katonah, New York, the office is within walking distance of the Metro North train station and easily accessible from all major driving routes including the Saw Mill Parkway and 684.
Privacy
All information disclosed within sessions is confidential as outlined in the HIPAA Notice of Privacy Practices for medical care. Patients will be asked for permission to have me communicate with other treatment team members, including physicians and psychotherapists as appropriate.
Payment and insurance information
Payment is requested at time of service by cash, credit card, or check made out to Amy Peck. Patients will receive a receipt to submit to insurance for possible reimbursement, but should verify benefits for medical nutrition therapy. Parents may set up a monthly billing cycle for teens and special payment plans are available upon request.
Cancellations
A minimum of 24-hour notice is required to reschedule or cancel an appointment unless it is a true emergency. The full fee will be charged for missed sessions without such notification. Please inform me if you would like a reminder phone call or email prior to your appointments.
